Incredibly serious case of constipation. So much, that her soft poop from above is leaking around her very hard plug of poop. Some people end up in the hospital when it gets this difficult. A great video.
Reminds me of a female UK univeristy student who regularly posts on Toiletstool.com. She suffers from chronic constipation and says that her turds get sucked back up if she relaxes on bearing down.

In the hospital, she would have been given a mineral oil enema to soften that up, then the plug of poop would have been removed by finger removal. I would have been happy to help this poor young woman as well.
This actually happened to my sister in law. She gave graphic details on how they extracted her impacted turd. Guess who had to excuse himself after for a stroke? :D
